Request a Library Card

This form is submitted to the OWWL Library System to request participation in the OWWL shared library card program.
Please use this form only if you do not already have an OWWL library card. You may use this form to request a card if you:
  • Are a full- or part-time resident of Ontario, Wayne, Wyoming, or Livingston Counties;
  • Attend school in these counties;
  • Pay property taxes in these counties;
  • Or were instructed to use this form by your library.
Submission of this form does not itself create a library card or grant borrowing privileges at any individual library
An OWWL library card provides access to collections and services at all 42 member libraries in the OWWL Library System, subject to system-wide policies.
Individual member libraries may have local circulation rules or service policies that apply when using materials or services at that location.
Your home library is the OWWL member library that serves the area where you live. It is used to determine eligibility and does not limit access to physical materials in other OWWL libraries.

  • My information will be reviewed to determine eligibility for an OWWL library card.
  • If a library card is issued, my use of library materials and services will be governed by OWWL system-wide policies and applicable local circulation rules at participating libraries.
